Degatide
Introduction:
Degatide contains the drug degarelix which is chemically a peptide , is given as a subcutaneous injections on the anterior abdominal wall. When degarelix comes in contact with the dermis, it can trigger an immune reaction characterized by one / more of pain ,erythema ,edema ,induration and nodules at the injection site. This is called as an "injection site reaction".
General aseptic procedures:
- Wash hands with soap and water before performing the injection procedure
- After washing hands,wear sterile gloves and keep them on for the duration of the procedure
- Disinfect the seal covering the mouth of the vial of Degatide (80/120mg) with spirit, allowing it to evaporate
- Reconstitute the vial as described below
- Give the injection of Degatide immediately after reconstruction
Starting dose/ initiation dose:
- Androgen deprivation therapy with degarelix is started with this dose.It consists of 240 mg Degarelix.
- This 240 mg is given as 2 separate injections of 120 mg Degarelix. Each vial of Degarelix 120 mg (40mg /ml) is reconstituted with 3 ml of sterile water for injection, gently swirl the vial to reconstitute, do not shake vigorously as it forms bubble .
- Formation of a few bubble is permissible. After reconstitution, Degatide should be a clear liquid free of any undissolved matter.
- It is given every 28 days. Starting 28 days after the initiation dose.It consists of a single injection of 80 mg Degarelix.The vial of Degarelix 80 mg is reconstituted with 4.2ml of sterile water.
- Injection degatide is to be given subcutaneously on the anterior abdominal wall only. This is because the anterior abdominal wall has thicker underlying subcutaneous tissue which reduce the possibility of extravasation of the injected drug into the superficial tissue planes
- Choose a site away from the areas of pressure such as areas where the belt (or) waistband is worn, over and around the ribs
Warning Signs:
- Hypersensitivity reactions
- Effect on QT interval (prolonged)
- Change in bone density
- Glucose tolerance
- Cardiovascular disease
- Laboratory test- Serum PSA, Serum Testosterone
